New Clients
Welcome! Prior to your first appointment, please print the intake form below, fill it out to the best of your knowledge and bring it with you. Plan to arrive 10 to 15 minutes early so we can review this form together and discuss a plan for your massage.
If you are acutely ill, massage should be postponed. If you have any chronic illness, recent surgeries, are taking prescription medication, and have any question whether massage would be safe for you, please consult with your physician prior to your appointment to obtain medical clearance. Please feel free to call me in advance, with any questions or concerns.
Prepare for your First Appointment

Bring the form with you and arrive 10 minutes early to your appointment.
First Visit
We will begin our first session with a brief conversation. In order to tailor your massage especially for you, we will go over your intake form together and I will ask about any current areas of tension or pain in your body that may need special attention, or any areas you would like to avoid. I will ask about any experience with massage and your anticipated preference for light, moderate or deep pressure. We will discuss your goals for your massage and try to answer any questions you may have. When our plan is complete, I will leave the room to allow you to undress to your level of comfort and get settled under the sheets and blanket on the massage table. I will knock before entering to make sure you are ready to begin. We will start any future sessions with a similar “check-in” conversation.
Your Massage
This is your time to relax. After I re-enter the room, I will do my best to make sure you are comfortable. I will make minor adjustments as needed with positioning, bolsters and pillows. I will only undrape the area of the body I am currently working with, one area at a time so that most of the body is warmed and covered. Private areas of the body will remain covered at all times. In order to get the most from your massage, it is recommended that you stay as relaxed as possible and try not to help me in my work. I may move your limbs into gentle stretch positions and it is best to stay relaxed and limp as possible.
I would like to assure you that although it is fine to chat, you may find it most therapeutic to let yourself be quiet and tune into your body. Sometimes the experience of relaxation will allow people to release emotion, to laugh, cry, or even fall asleep. These responses are normal and appropriate. This is a safe time and space to let your mind and body unwind and let go of physical and emotional stress, pain and tension. Your session is confidential and judgment free. The massage and energy work I offer is intended to help you engage with your body and mind in a powerful and positive way, stimulating your body’s own healing mechanisms.
After the Massage
When your massage comes to an end, I will leave the room to allow you to slowly get up and get dressed. I will ask you to open the door when you are ready and I will reenter the room. We will end our session with a brief conversation about how the massage went for you, how you are feeling and what you might expect. This conversation will help me gage your experience with the treatment and help me in meeting your needs in future sessions. We will book any future appointments as desired and I will take payment at this time. Payment methods can include cash, check or charge. Health savings plans are typically accepted.
